Life care planning is a vital part of long-term care and treatment and has been since the 1970’s when it emerged as a preeminent case management tool. As time went on, it became clear that a well-written Life Care Plan (drafted by an experienced and well-trained medical doctor) can be a “single source of truth” for all interested parties.
